How to Make an Appointment
STEP ONE: Review the information on the website and FAQ page. This information will answer most of your initial questions.
STEP TWO: Request Living Matrix Intake Forms. This can be done by calling our office at (360) 457-5139.
If you have records from other providers, please use the release form above to request those records to be sent to us. Please consider completing those requests as soon as possible, since it may take your other providers some time to process your records request(s).
STEP THREE: Once your Intake Forms are completed and received by our office, a staff member will call you to schedule your new patient appointment. (Once you complete each form within the Living Matrix system, it will automatically be sent to the provider. You do not need to take any additional steps to submit the forms to our office.)
